- Copy of CURRENT Utility Bill (Elec, Gas, Water)
- Copy of Photo ID
- If you ARE the Renter:
- Copy of Signed & dated Lease
- Copy of Utility bill (Elec, Gas, Water)
- Copy of Photo ID
- If you LIVE with FAMILY or FRIENDS
- Affidavit of Homeowner – NOTARIZED (Under Forms)
- All items under “A”
- Affidavit of Parent – NOTARIZED (Under Forms)
- Parent’s Current bill from the USPS
- Parent’s Photo ID
- If Student attended another school
- New Jersey State Transfer Card (Under Forms)
- Records Request Form (Under Forms)
- Last Report Card or Transcript
- Birth Certificate = Student
- Immunization Records = Student
- Universal Health Record = Students (Pre-K & Kindergarten ONLY)
- Custody Documents – If required, showing docket number & Judge’s signature
- Certificate of Transportation – if pickup and drop off are not at child’s home. This form is to be NOTARIZED by all parties. (Under Forms)

IMPORTANT THINGS TO REMEMBER
Pre-K Registration
- K-3 Child must be 3 years of age on or before 10/1/2023.
- K-4 Child must be 4 years of age on or before 10/1/2023.
- A Universal Child Health Record form MUST be completed by child’s health care provider
- Flu shots ARE REQUIRED for all pre-school students ages 6-59 months by December 31
Kindergarten Registration
- K-5 Child must be 5 years of age on or before 10/1/2023.
- A Universal Child Health Record form MUST be completed by child’s health care provider

Registration applications that are lacking the required information and/or documentation will NOT be processed until the information and/or documentation is received.
